We provide information, education and access to health care services in a welcoming and trusting environment. We support prevention, wellness and Board- Certified primary medical care for all without regard to income, ethnicity or gender.
We support our patients in overcoming their barriers to care. We empower them to become advocates for their own health and wellness, as well their community.
Our Mission
Urban Solutions provides health information, education, prevention and medical care services “across the life span” to the medically, economically, culturally and educationally disadvantaged without regard to income, ethnicity or gender. Founded by a team of committed medical activists, the organization embraces a unique medical outreach model, which forges an active partnership between our primary care medical providers and the community they serve through providing access to a full circle of care.
Our History
Urban Solutions was founded as a nurse managed practice in 1993 by CEO Yvonne S. Jones, BSN. It was conceived as the charitable medical mission of Health Associates of South Philadelphia, a community-based primary care practice. In response to the South Philadelphia community’s urgent need for education, prevention, wellness and outreach services which were not funded by traditional health care insurance. The agency obtained non-profit status in 2000. Although our roots remain in South Philadelphia, our services are now accessible city-wide.
Urban Solutions focuses on eliminating barriers to care for vulnerable minority and low-income populations impacted by poverty, lack of insurance, addiction, and high-risk sexual behaviors. These conditions result in chronic illnesses such as hypertension, diabetes, asthma, sexually transmitted diseases and HIV/AIDS, for which ongoing education is essential to prevention and treatment.
A hub of community care, Urban Solutions acts as the prevention, education and medical outreach arm of Health Associates of South Philadelphia (“HASP”), which is staffed by six Board-certified adult and pediatric medical providers. Urban Solutions provides health education, patient advocate and outreach services to the practice’s 18,000 adult and pediatric patients, serving another 4,000 patients through specialized clinics and programs, and hosting annual community health events for teens, adults, seniors, men and the medical community.
Grounded in the South Philadelphia community for over 20 years, HASP and Urban Solutions are viewed by multiple generations of families as safe places to find trusted medical advisors. Urban Solutions, Inc. is the only community-based, African American controlled primary care provider which receives federal Ryan White Care Act Title I funding through The City of Philadelphia Department of Health and Title X funding through The Family Planning Council of Pennsylvania.
